Uralla Community Care supports people who are eligible for government-funded care, support or transport services. This may include people who have been approved for support through the Commonwealth Home Support Programme, Support at Home or Community Transport programmes.
Eligibility can depend on your age, circumstances, care needs and assessment outcome. If you are unsure whether you can access services, our team can talk you through the general process and explain where to begin.
We provide services across Uralla, Armidale and Walcha.
For most Australian Government-funded aged care services, you will need an assessment through My Aged Care before services can begin.
The assessment is used to understand your situation, what you may need help with and which supports you may be eligible to receive. Once your assessment is complete, you may be given a support plan or referral codes for approved services.
The Commonwealth Home Support Programme, often called CHSP, is accessed through My Aged Care.
You can contact My Aged Care to request an assessment. If CHSP support is approved, you may receive referral codes for the services listed in your support plan. These referral codes can then be provided to a service provider, such as Uralla Community Care, so the service can be discussed and arranged.
The Commonwealth Home Support Programme provides entry-level support for people who need some extra help to remain living at home and taking part in everyday life.
It is designed for people who may only need a small amount of assistance, rather than a higher level of ongoing care. Depending on your assessment, CHSP may include support with daily tasks, transport, social connection or other approved services.
Starting with My Aged Care, referrals and different programme names can feel confusing at first.
